1. Full payment is required at time of registration. This includes late fee if received after May 31. See rates below. Rates have been raised due to rising costs and declining collections.


2. In-person registration is Sunday April 29 and in the Religious Formation Office, also during office hours: Until May 3, Mon., 1 pm to 8:30 pm, Tue. and Wed., 10 am to 5:30 pm, Thur., 10 am to 3 pm, Closed Fridays. Summer hours from May 7 to June 30. Mon. though Thurs. 10 am to 3 pm.
   


3. We are enrolling your child/children for the same day in the next grade level. If you wish to change days please indicate the desire day next to the child's name. NOTE: We encourage 7th & 8 th level students who do sports to come on Monday evening.


4. Please make sure all information on the form is correct, including child's grade and school and your email address. Email will be our primary way of communicating with you during the year. Mail the form with your check in the enclosed envelope.


5. If you are enrolling a child for the first time, you must come into the office to fill out a blue form and bring your child's Baptismal Certificate. You may not add a name to the re-registration form.

Early registration: by May 2, 2012:

• Receive a 10% discount if form and fee are received by May 2.
• Mail the enclosed forms or drop them by our office by May 2.

Late Registration:

• Registration received after May 31th will be subject to a 10% late fee.
  

 Requests for particular catechists or classmates must be received by May 31. If you want your child with a particular child both children must be registered by May 31 in order to accommodate your request
